By submitting this registration form and attending the trial, the Trialist acknowledges and agrees to the following:
No Guarantee of Selection
Participation in the trial does not guarantee selection, registration, placement, or the offer of a playing position within any Port Melbourne Sharks SC team or program.
All selection decisions are at the sole discretion of the Club and its coaching staff.
Conduct During the Trial
The Trialist agrees to conduct themselves in a respectful, safe and sportsmanlike manner at all times.
Any reckless, dangerous, violent, abusive, malicious or unsporting conduct directed towards another player, trialist, coach, official or spectator may result in immediate removal from the trial and exclusion from future consideration.
Assumption of Risk
The Trialist acknowledges that participation in football training and trial activities involves inherent risks, including but not limited to physical injury, illness, disability or property loss.
The Trialist voluntarily accepts these risks associated with participation in the trial.
Injury Disclaimer
Whilst Port Melbourne Sharks SC will exercise reasonable care and diligence in conducting the trial, the Club, its officials, coaches, volunteers and representatives shall not be responsible for any injury, loss, damage or expense sustained by the Trialist arising from participation in the trial, except where required by law.
Fitness to Participate
The Trialist confirms that they are physically fit and medically able to participate in football activities and have no current injury, illness, medical condition or physical limitation that would prevent safe participation.
The Trialist agrees to immediately notify the Club if any injury or medical condition exists or develops prior to or during the trial.
Accuracy of Information
The Trialist warrants that all information provided in the registration form is true, accurate and complete.
The Club reserves the right to withdraw a trial invitation or remove a Trialist from consideration where false, misleading or incomplete information has been provided.
Medical Treatment in an Emergency
In the event of an accident, injury or medical emergency, the Trialist authorises Port Melbourne Sharks SC to obtain such medical treatment as may be considered reasonably necessary and acknowledges that any associated costs remain the responsibility of the Trialist or their parent/guardian.
Photography and Video Recording
The Trialist acknowledges that photographs and video recordings may be taken during the trial for player assessment, coaching, promotional or administrative purposes.
By attending the trial, the Trialist consents to such recording and use by the Club.
Privacy
The Trialist acknowledges that personal information collected through this registration process will be used by Port Melbourne Sharks SC for trial administration, player assessment, registration and football operations purposes and will be handled in accordance with applicable privacy laws.
Acceptance
By submitting this registration form, the Trialist confirms that they have read, understood and agree to these Terms and Conditions.
